Open to Bergen County and Rockland County residents 18 years of age or older as of the date of this entry. This contest is being held from February 22, 2008 to April15, 2008. All entries must be received during the contest period. Only one entry allowed per person during the contest period.
2. To Enter: Complete the entry form at www.bergenhealthandlife.com. Your entry must be received no later than April 15, 2008. No correspondence will be answered. Bergen Health & Life will not be responsible for lost, late, misdirected, incomplete, incorrect or otherwise undeliverable entries.
3. Prizes: On or about April 16, 2008, a random drawing from all eligible entries will be conducted. All decisions are final. The following prize(s) will be awarded: one $200 gift certificate redeemable at Bruno Jamais, 24 East 81st Street, New York, NY 10028. Odds of winning depend on the number of eligible entries received. Winner will be notified by phone. Prize(s) must be claimed no later than April 30, 2008. Prizes not claimed within such time will be given to an alternate winner. Prize(s) are not transferable and may not be given, bartered or sold. There are no cash prizes or prize substitutes. Winners are responsible for all local, state and federal income taxes associated with winning the prize(s).
4. Limitations of Liability: Entrants agree to be bound by these rules and that the decision regarding the winner is final.
